How to Request Funding

To apply for funding, an organization whose purpose is consistent with the objects of the The Forzani Group Foundation must submit a written proposal on the organization's letterhead. Please include the following information:

  • Charitable registration number
  • Amount of funding requested
  • Geographic area served by the organization
  • Brief description of the project including objectives, budget, target groups to be served, needs that will be addressed, activities to be undertaken, expected outcomes
  • Time frame for which the financial support is requested i.e. one-time grant, ongoing support or special project
  • Other sources of funds applicable to the project
  • Copy of the latest annual report and latest audited financial statements
  • List of board of directors and officers

Requests for support may be submitted at any time and are reviewed (2) two times per year. If more information is needed, an officer will contact the organization. During the review process, the officer may arrange a meeting or site visit to discuss the project. When the review process is complete, the officer will make a recommendation to the Foundation's Board of Directors.

  • January 1st - 31st
  • June 1st - 30th

The Forzani Group Foundation receives more requests than it can support. A decision to decline a request does not imply that the applicant's program is not needed or worthy, but simply that it does not fall within our giving guidelines, priorities or that funds are not available.

Only those who qualify based on information provided in the donation request form will be contacted.
